Care Partners Care Coordination

• Care Partner’s Care Coordinators can help clients connect with area service providers, develop a plan for in-home, long-term and/or advanced care, and receive continuing encouragement and support
• End of life assistance includes helping clients understand and complete a Health Care Directive, locate and coordinate local end of life resources, and communicate wishes to the family
• There is no charge for services, but clients may be invited to make an optional donation based on household income

Aging Well Resources Care Navigator

• The Care Navigator provides resource information and referral assistance for area services and programs
• Helps clients connect with care coordinators for ongoing support and guidance
