Care Partners Care Coordination

  • Care Partner’s Care Coordinators can help clients connect with area service providers, develop a plan for in-home, long-term and/or advanced care, and receive continuing encouragement and support
  • End of life assistance includes helping clients understand and complete a Health Care Directive, locate and coordinate local end of life resources, and communicate wishes to the family
  • There is no charge for services, but clients may be invited to make an optional donation based on household income

Care Partners Companion Volunteers

  • Trained, background-checked volunteers connect with older adults to touch base, share friendly visits, provide respite for caregivers, or offer support at end of life. 
  • Companions often meet weekly but are free to determine frequency and timing that works for them
  • Arrangements can be made to chat by phone, meet in clients’ homes, or enjoy walks or outings together

Cook County End of Life Doulas

  • Trained Doulas provide emotional, social, and spiritual end-of-life support for individuals and their families at any point in their journey – from early diagnosis to last breath and anywhere in-between
  • Services include assistance with creating and sharing life-stories, advanced care planning, memorial planning, grief support, being present at end of life and more

North Shore Health - Home Health Care

  • Provides care services to individuals in their place of residence for the purpose of promoting, maintaining, or restoring health or minimizing the effects of illness and disability
  • Offers short-term and long-term care (including post-surgery), which may incorporate RN skilled nursing, home health aides, or physical therapy
  • Examples of assistance include nursing assessments, assistance with bathing/hygiene, medication management, wound care, IV therapy/management, end of life care, and foot care
  • Home Care will discuss payment options and check with insurances to see if the client qualifies for Home Care benefits
  • Accepts Medicare, Medicaid, other insurances, VA and Sliding Fee

Weaving Pathways is a culturally responsive approach to end-of-life planning developed through a collaboration between Honoring Choices/Light the Legacy and the Center of American Indian and Minority Health (CAIMH)

For information regarding Provider Orders for Life Sustaining Treatment (POLST),  a portable medical order that can give patients with advanced serious illness the option to exercise increased control over the treatment they do and do not want to receive at the end of life.
