In addition, we use Google Forms to capture and transmit program information requests from our website to a select group of local organizations. These forms require you to provide contact information including your name, address, phone number and email address. We also require you to identify your preferred method of contact and to briefly indicate the nature of your inquiry. All data collected from these forms is used to help the organization from whom you are requesting information respond to your inquiry.

We are not responsible for the privacy or information sharing practices or data retention policies of the organizations with whom we share your information through Google Forms, and we recommend that you contact those organizations with any questions regarding their privacy and information sharing practices and/or data retention policies.
