Lunch & Learn – Understanding POLST (Provider Orders for Life-Sustaining Treatment)
October 28 @ 12:00 pm - 1:00 pm

Care Partners and Cook County Higher Education are pleased to present a Lunch & Learn series designed to support aging well in Cook County! Presented on the fourth Wednesday of the month from noon to 1pm at Cook County Higher Education, these free Lunch & Learn classes feature speakers from area organizations like Sawtooth Mountain Clinic, AARP, Arrowhead Area Agency on Aging, and more! You can join online OR in person at Cook County Higher Education. Thanks to the generosity of individual donors and granting organizations, a light lunch is provided free of charge for those who are able to attend in person.
In this session, Dr. Kurt Farchmin, geriatrician and former Sawtooth Mountain Clinic provider, will help us understand one of the most frequently misunderstood Advance Planning documents – the POLST form. He’ll walk us through the form step by step, explain what it is, why it is important, and how to work with your medical provider to complete it. Dr. Farchmin will also touch on the crucial role of Advance Planning documents from a medical provider’s perspective. Register here!
